SPECIAL POWER OF ATTORNEY

KNOW ALL MEN BY THESE PRESENTS:

residents of ________City to be our true and lawful Attorney-In-Fact, for us and in our name,
place, and stead, to do and perform the following things and acts, to wit:

termination of the case which includes but not limited to conferences, hearings, court
mediations, preliminary conference, pre-trial, mediations through PMC and all
subsequent hearings/meetings requiring my attendance;
2- To discuss possibility of an amicable settlement, negotiate, enter into, conclude, execute,
sign, and deliver a compromise agreement as she may deem just and equitable;
3- To submit the case for arbitration, simplify the issues, amend and/or agree to amend the
pleadings, obtain stipulations of facts and of documents to avoid unnecessary proof, to
limit the number of witnesses, and agree on such matters and do such things as may aid
in prompt disposition of the action.
4- To sign, execute deliver and receive documents, agreements and other writings of
whatsoever nature, kind and description and other legal papers pertaining thereto
necessary to carry out the foregoing authority including Affidavits, Verifications, etc. To
file petition or motion/s before the Court of Appeals or the Supreme Court.
5- To do other acts relative thereto;

To sign, execute, deliver and receive on our behalf documents, agreements, processes
and other acts relative to the said complaint.

To perform all acts necessary to achieve the above-mentioned purpose.

        HEREBY GIVING AND GRANTING unto our said attorney full power and authority
whatsoever requisite or necessary or proper to be done in and about the premises as fully to
all intents and purposes as I might and could lawfully do if personally present, with power of
substitution and revocation, and hereby, ratifying and confirming all that our said attorney or
their substitute shall lawfully do or cause to be done under and by virtue of these presents.
This power of attorney is subsisting and valid until revoked.
