1. By this LOU, the Party acknowledges that his/her admission and enrolment at Adamson University is only temporary
and unofficial unless he/she fully complies with all the requirements necessary for his/her official admission at
Adamson University.

2. By this LOU, the Party undertakes to submit the following lacking requirement/s on or before JULY 15, 2022,
namely:

For Freshmen Student:

 Original copy of Grade 12 Report card/F-138
 Original copy of Certificate of Good Moral Character
 Original copy of PSA Birth Certificate
 Two (2) pieces 2 x 2 ID picture

For Transfer Student:

 Original copy of Transfer Credentials / Honorable Dismissal
 Original copy of Certificate of Good Moral Character
 Original copy of True Copy of Grades
 Original copy of PSA Birth Certificate
 Two (2) pieces 2 x 2 ID picture

3. By this LOU, the Party agrees that should he/she fail to submit the lacking requirement/s on the date above specified,
his/her temporary enrolment at Adamson University maybe cancelled at the discretion of the school.

4. By this LOU, the Party acknowledges that during his/her temporary enrollment, the Certificate of Enrollment and
Assessment of Fees shall become available only upon submission of all the hard copy original mandatory requirements;

5. By this LOU, the Party acknowledges that cancellation of his/her enrolment due to his/her failure to comply with the
submission of the above requirements shall mean forfeiture of whatever has been paid representing his/her tuition and
miscellaneous fees;

6. By this LOU, the Party acknowledges and represents that he/she is executing this LOU in good faith and with full
intention of complying with each and every provision detailed herein.
