Affidavit of Loss

I, JASON ODON, of legal age, single, and residing at Blk 31 Lot

20 Navo Homes, Brgy. Tanza 2, Navotas City after being sworn to in
accordance with law, depose and say:

I am a registered voter of Brgy. Tanza 2, Navotas City,

Philippines and I was issued an identification card which I usually keep
in my wallet, which in turn, I always bring with me wherever I go and
for identification purposes in every transaction I make;

That due to the circumstances that cannot be prevailed, I have

found lately, my office ID was nowhere to be found;

That I have already exerted my thorough search in order to find

the said ID, but to no avail;

That I am executing this affidavit to attest to the truth above-

stated, and for whatever legal intents and purposes it will serve.

IN WITNESS WHEREOF, I have hereunto set my hands this 17th

day of August 2022, in Navotas City, Philippines.
