PERFORMANCE
Prior to performing the procedure, introduces self and verifies the client's identity

a. Explains to the client what you are going to do, why is it necessary, and how he or
she can participate.
b. Discusses how the effects of the oxygen therapy will be used in planning further
care or treatments.

Good morning maam, I am Wendrea your student nurse for today. Can you please tell
me your name maam and also your birthdate? Im just going to check your wristband
maam? Okay, So right now maam. I’ll be giving you oxygen through this nasal cannula
to improve your oxygenation and to help you breathe better. This will be place on your
nose maam so u can still eat and drink rin naman using your mouth.
Performs hand hygiene to prevent microorganisms and observes other appropriate

infection prevention procedures.
3 Provides client privacy, appropriately. To provide client comfort.
4 Setting up the oxygen equipment and the humidifier:
a. Attaches the flow meter to the wall outlet or tank. The flow meter should be in the
off position.
b. If needed, fills the humidifier bottle. (done before coming to the bedside)
c. Attaches the humidifier bottle to the base of the flow meter. To Providing comfort to
the patient reduces dryness
d. Attaches the prescribed oxygen tubing and delivery device to the humidifier. So that
you can only give the exact prescribe o2 to the client and to assure that he/she will not
get oxygen toxicity
5 Turns on the oxygen at the prescribed rate and ensures proper functioning.
a. Checks that the oxygen is flowing freely through the tubing. If there is any leakage
to the tubing replace it.
b. Sets the oxygen at the flow rate ordered.

This to make sure there is air flowing through the tube and the device functioning.
6 Application of the appropriate oxygen delivery device:
a. Puts the cannula over the client's face, with the outlet prongs fitting into the nares
and the tubing hooked around the ears.

(now sir I will pe putting this nasal cannula to your nostrils sir.)
b. If the cannula will not stay in place, tapes it at the sides of the face.
To avoid cannula from moving all over the place.
c. Pads the tubing and band over the ears and cheekbones as needed. To minimize the
irritation and pressure.
7 Assessment of the client:
a. Assesses the client's vital signs, level of anxiety, color, and ease of respirations, and
provides support while the client adjusts to the devices. To make sure the client
responding positively to the therapy administered.
b. Assesses the client in 15 to 30 minutes, depending on the client's condition. To see if
there is any unussualities.
c. Assesses the client regularly for clinical signs of hypoxia, tachycardia, confusion,
dyspnea, restlessness, and cyanosis. To determine if the patient needs a higher level of
oxygenation.
d. Reviews oxygen saturation or arterial blood gas results if available. To prevent
complications.
8 Assessment for Nasal Cannula:
a. Assesses the client's nares for encrustations and irritation. To relief if there is any
irritations.
b. Applies a water-soluble lubricant as required to soothe the mucous membranes.
c. Assesses the top of the client's ears for any signs of irritation from the cannula
tubing. If present, padding with a gauze pad may help relieve the discomfort.
9 Inspects the equipment on a regular basis.
a. Checks the liter flow and the level of water in the humidifier in 30 minutes and
whenever providing care to the client.
b. Makes sure the safety precautions are being followed. To reduce risks of life.
10 Performs hand washing/hand hygiene and aftercare.
Documents findings in the client record using forms or checklists supplemented by

narrative notes when appropriate.
