REVOCATION OF POWER OF ATTORNEY

KNOW ALL MEN BY THESE PRESENTS:

Power of Attorney, named and appointed (name of old agency), as my true and lawful attorney-
in-fact, for the purposed and with the powers set forth in the instrument;

Employment Administration (POEA) as a requirement for accreditation purposes;

WHEREAS, I intend to revoke and terminate the said Power of Attorney and render
without further force and effect;

countermanded, annulled and made void and by these presents do revoke, countermand,
