Dear Ma’am/Sir:

I am writing this to follow-up on the letter dated 14 March 2022, which I previously
sent to your office concerning our intent to terminate the follow PLDT Enterprise subscription:

In the aforementioned letter, we stated that the last billing period (for the month of
March 2022) will be settled within ten (10) days from the receipt of the Billing Statement. On
March 15, 2022, we sent an email concerning this request of termination and asking for the
duration of its effectiveness. In reply, Eugene Masaga of PLDT Enterprise Care sent an email
dated April 19, 2022, stating that the termination of this account was in progress with
Provisioning Request Number: SMEKOR20220318408898. However despite this email, PLDT
has yet to follow through with the said concern for almost five (5) months as evidenced by the
PLDT Bills Online for Customer No. 652567843 for the month of July 2022 has a negative
balance of Php -3,520.11. Thus, we strongly demand that this account be properly terminated.
Attached herewith are the relevant are the letter and the Billing Statements for your perusal.
