Dear Ma’am/Sir:

I am writing this to follow-up on the letter dated 14 March 2022, which I previously
sent to your office concerning our intent to terminate the following PLDT Home subscriptions:

In the aforementioned letter, we requested for the termination of the accounts stated
therein and that the corresponding outstanding balance will be settled within ten (10) days of
receipt of the Billing Statement. However, these two accounts were the only ones among the
five that are yet to be terminated. A month after the balance was settled, a Bill Statement that
has a balance stated in the negative was sent for the month of April 2022. Following that, a Bill
Statement for the month of July 2022 stated that the accounts were charged with the amount
of Php 9,321.57. This caused great concern since the letter we sent clearly stated that these
two were included for the request for termination. Thus, we reiterate the request for the
termination of the aforementioned accounts. Attached herewith are the relevant are the letter
and the Billing Statements for your perusal.
