those associated with the camps to administer general first aid
treatment for any injuries received to my child during the camp.
If their injury sustained is life threatening, or in need of emer-
gency treatment, I authorize the camp or its representatives to
summon any or all professional emergency personnel to attend,
transport and treat my child.
If the injury sustained requires hospitalization, I understand
that I, or medical insurance company is solely responsible for
all bills and claims that may be filed as a result of any injury.
By signing this medical release and liability form, I understand
that I will not hold Chrissy Mulvihill or Heritage & their represen-
tatives responsible for any injury sustained to my child or for
any other reason while my daughter is participating in the
camp.

About the Coaches!

Chrissy Mulvihill has just completed her 6th year
as Head Coach at Heritage Elementary. Prior to
this she was Head coach at St. Jerome Elemen-
tary for 15 years. Her teams have been very suc-
cessful including many Diocesan Champion-
ships, 1994 Arizona Junior High State Champi-
ons, 2005 Arizona Junior High State Runner up
and PUSD 2010 runner up. She has also been
coaching club volleyball at different levels for the
past 9 years for Arizona Thunder and Club Ar-
rowhead. She currently is the PE coach at Heri-
tage Elementary.

*Kristen PreachHas played for Xavier Prep,
club volleyball, coaches club for AZ Storm and is
currently playing for Grand Canyon University.
*Lacey Preachplayed for Xavier Prep, club vol-
leyball and is coaching club for AZ Storm.

playing for Arizona State University.

school players from all over the central/west val-
ley with competitive club and school ball experi-

This session will focus on teaching

the basic skills of passing, serving,
court play and teamwork. We work
in small groups setting with a 5
player to 1 coach ratio. We will
have a round robin tournament on

and watch.

ing, serving, hitting, blocking, court
play and strategies of the game.
