CONSENT FORM

I voluntarily give to REX Group of Companies the right to make a thorough investigation of
my scholastic records, employment background, not limited to: the compensation package
(Copy of Payslip, BIR 2316 & COE) and community activities and agree to cooperate in such
investigation.

I agree to submit myself to pre-employment physical/medical examination and any other

examinations that maybe required by the Company. I understand that the entire contents of
this application form as well as the interview results, may be used by the Company in
whatever manner it may serve in connection with my application for employment.

I further understand that any false answer or statements made by me on this application or
any supplement thereto, or in connection with the above mentioned investigation, shall be
sufficient grounds for disqualification and subsequently for immediate discharge.

If hired, I will abide by the rules, regulations and policies of the Company and agree to follow
all management prerogatives as imposed for the best interest of the business.
I hereby certify that any and all information provided by the undersigned are true and
correct. Should any of the said information is proven to be false or fabricated after
background check or investigation, the same shall result in the cancelation of my job
application with the company and if subsequently hired, the same shall constitute a ground
for termination of my employment.
I, after being duly informed and/or notified of my basic right to privacy as well as the data
privacy rights under the Data Privacy Act (R.A. No. 10173) and its Implementing Rules and
Regulations, I hereby consent to the disclosure of certain personal information that will
serve the purpose of my application as ___________________. I hereby waive/relinquish such right,
claim or action that I may have under pertinent Philippine laws against Rex Book Store, Inc.,
its affiliates and agents that might arise from or that which might be due to such disclosure
of personally identifiable information.
