Davao City Subject: Proposed corrective actions in relations to notice of violations of PD 1586, RA 6969, RA 8749 and their Implementing Rules and Regulations Dear Dir. Bulacan: We are pleased to submit to your office our corrective actions in relation to the Notice of Violations receipt by this office, last August 8, 2022 with findings cited as follows hereunder: 1. PD 1586 A. Failure to submit Compliance Monitoring Report or CMR The DdOPH - Laak has already complied the Compliance Monitoring Report (CMR) for the 1* Semester, 2021, 2%! Semester, 2021, and 1* Semester, 2022 and had forwarded to the email address of our Managing Head for his approval. Then a notification confirmed that it ‘was successfully sent to the aforementioned for his action within 5 days as prescribed in their guidelines. Subsequently, a notice from =EMB through their email: emrnotification@emb.zov ph notifying the Managing Head that the said report is ready for approval. After the approval , the report was submitted immediately to EMB EIAMD for review. As to the status and updates, it can be validated and seen in the Compliance ‘Monitoring System, B. Improper labeling of generated M501 wastes DdOPH - Laak had already complied the proper labeling of generated waste as well the color coding of waste bags and trash bins. The hospital conducted a meeting last February 18, 2022 at DdOPH - Laak Admin Office regarding the importance of proper labelling of generated M501 waste as manifested in the minutes of the meeting. That there's a need to strictly implement the proper labeling of different waste according to categorization, waste segregation and color coding of waste bag/trash bin is need to be observed at all times in different areas of the hospital. That the hospital shall strictly implement the policies and guidelines on effective and proper handling, collection, transport, treatment, storage and disposal of health care wastes as articulated under DENR - DOH Joint Administration Order No. 2, Series of 2005. Park 4, Poblacion, Laak Davao de Ore PROVINCIAL ECONOMIC ENTERPRISE MANAGEMENT OFFICE DAVAO de ORO PROVINCIAL HOSPITAL ~ LAAK As a result, the hospital through our Health Care Waste Management Committee, Hospital Infection, Prevention and Control Committee, and Pollution Control Officer committed to strengthen this undertaking in adherence to the policies and guidelines of proper segregation and labeling of all hospital wastes. As well, to capacitate our Utility Workers as frontline personnel on this job. 2. Failure to comply with the proper storage and labelling requirements (RA 6969) DAOPH - Laak had already complied with the proper storage and labelling at the MRF or Material Recovery facility. ‘The health facility conducted a meeting last February 18, 2022, regarding improper storage and labeling of hazardous wastes, as manifested in our minutes of the meeting, wherein the hospital MRF should be compliant with the mandates as to proper storaging and labeling of of waste. The need to enhance the structure was recommended by the EMB staff during their visit . Lastly the hospital must strictly implement the policies and guidelines on effective and proper storage of hazardous waste, proper handling, collection, transport, treatment and disposal, and shall follow the rules and regulation of the EMB/DENR pursuant to Section 11.1 of DAO 2013-22. The Facility coordinated with LGU - Laak MENRO regarding the request for drums to be used as container to improve proper segregation of waste. As such, the LGU granted our request and provided us with 3 drums, last April 4, 2022 (On July 7, 2022, we conducted meeting on Waste Management Program at DdOPH - Laak Administrative Office together with the MENRO, PENRO and Provincial Engineering Office regarding the status of hospital waste and the MRF. As a result, there’s a need to re-construct, MRF to efficiently store all hospital waste with proper segregation prior to collection and transport to municipal garbage site with the agreement and condition that all infectious and hazardous waste are properly segregated and disinfected In addition, some hazardous wastes are not yet hauled because the LGU of Laak vehicle has no permit to transport hospital waste .On the other hand the treatment facility of the province installed at DDOPH - Montevista is not yet operational. Currently, the hazardous waste was stored in the facility. As a results, there is a need for collaboration and a conduct a meeting for the 4 DdOPH provincial hospitals regarding the hazardous waste transportation and ‘treatment. Last July 12, 2022, the 4 DdOPH hospitals including PEEMO, PENRO and PEO conducted a meeting at the PGO Conference Room regarding issues on hospital waste. Two transport vehicles were provided for the collection and disposal of wastes. A recommendation coming, from Provincial Engineering Office regarding the improvement of road going to the MRF for easy access of LGU garbage vehicle. Then the plotting of schedule for the 4 Provincial Hospitals as to the collection and transportation of waste. Lastly, the PENRO advised that each health facility must apply TSD or Treatment, Storage and Disposal particularly Purok 4, Poblacion, Laak Davao de Oro PROVINCIAL ECONOMIC ENTERPRISE MANAGEMENT OFFICE, DAVAO de ORO PROVINCIAL HOSPITAL — LAAK hazardous waste and Permit to Transport of hazardous waste. As a respondent, our hospital committed to comply all the requirements assigned to us. 3. Non-submission of Source Emission Test and Monitoring Report (RA 8749) DdOPH - Laak had already coordinated with BSI or Berkman System Incorporated, the Environmental Management Service Provider last year 2021, then the BSI forwarded Source Emission Monitoring Proposal to DdOPH - Laak last November 18, 2021, the firm is waiting for the schedule of Emission test/monitoring and budget for the cost of services for the sampling activity worth of 42,560.00 inclusive of 12% vat. On May 16, 2022 our facility has submitted the Test Plan for the proposed Source Emission Test to be conducted by BSI at the EMB office. Subsequently, the Source Emission Test/Monitoring was conducted last May 26, 2022. On July 4, 2022 we already received the final report/results of the source emissions monitoring from BSI and we already provided a copy of Emission Test Results to EMB email address: embxirecords@emb. gov. ph.
