Nursing Theory: Scenario 5 (Visual Deficit)

Mrs. Z. is a retired schoolteacher and an avid reader. She states, “I read very little these days
because I have difficulty reading normal print.” She has worn the same glasses for the past 10
years and has not seen an eye doctor for years. Her glasses are in bad condition with soiled and
scratched lenses.

1. Based on this scenario, develop an SBAR report to the social worker.


2. Develop a teaching plan for Mrs. Z.
3. Class presentation: SBAR and teaching plan.

1. SITUATION
- My name is Alexis Delgado and I am the nurse of Mrs. Z
- I am writing this report about Mrs. Z because I am concerned that she is having trouble seeing,
which can result into many other issues that should not be happening.
- Loves to read but hasn’t been able to read normal print anymore
- Worn the same glasses for the past 10 years which are in really bad shape.
- BACKGROUND
- Mrs. Z is a retired schoolteacher who also loves to read, but for the last 10 years has not been able
to see very well.
- ASSESSMENT
- During the assessment it is clear that Mrs. Z cannot read any of the paperwork that has been given
to her even with her glasses on. I have assessed that the client’s glasses are scratched and dirty.
When I asked when the last time she saw an eye doctor she said it has been years, and the glasses
that she has now are over ten years old.
- RECOMMENDATION
- It is recommended for Mrs. Z to go see an eye doctor to get a new prescription, as well as a new
pair of glasses.
- Doing this will allow her to be able to read the fine print of her books again.
2. Teaching Plan
- A teaching plan for Mrs. Z would be to make sure that she is going to see an eye doctor yearly,
instead of every 10 years.
- If her glasses are broke or scratched, get new ones
