INTERNET SERVICE SUB CONTRACT

This Internet Service Agreement (hereinafter referred to as the “Agreement”) is entered and shall be
effective as on July 2021, By and Between

MICHAEL VINCENT LIM, represented by CAROLINE LIM (hereinafter referred to as the

“PROVIDER”), having its registered place of business at NAGA CITY, and;

Maria Gloria Claveria (hereinafter referred to as the “USER”), having its location at Canaman
Naga City, Philippines.

The PROVIDER and the USER are collectively referred to as the "Parties" and "Party" individually.

WHEREAS, the PROVIDER has entered with CONVERGE, internet provider for the property being
rented by the USER at CROSSANDRA STREET, DEL ROSARIO NAGA CITY.

WHEREAS, the USER wishes to engage / continue the internet services from the PROVIDER
existing contract with CONVERGE.

AND WHEREAS, the PROVIDER has agreed to extend the necessary services to the USER

NOW, THEREFORE, in consideration of the mutual covenants and promises made by the Parties
hereto agree as follows:

The initial term of this Agreement shall be effective from the date June 24, 2022. The term of the
Agreement shall be for a period of one year [co-terminus with the rental agreement]. The payment
for internet services amounts to ONE THOUSAND FIVE HUNDRED PESOS [P1,500.00] MONTHLY
without the need to demand from the PROVIDER.

The USER shall give outright the PROVIDER, THREE THOUSAND PESOS [P3,000] representing
One [1] month deposit and One [1] month advance*, applied on the first month for the internet
service contract.
The PROVIDER shall provide the USER – Special Power of Attorney to represent the PROVIDER
with CONVERGE, who shall be responsible for the internet services and maintenance and technical
support of this agreement.
The Agreement shall be terminated if any of the following events occur:
1. The USER does not make the payment on time;
2. The PROVIDER shall terminate the contract with CONVERGE to provide the necessary
internet services to the USER;
3. Either Party breaches the contract or written notice is given to either of the party;
IN WITNESS THEREOF, the Parties hereto have executed this Agreement as on the day and year
mentioned above.
