I, EDWARD DELA VEGA ROLDAN, of legal age, Filipino citizen and

resident of  726 Tramo Road., Manuyo Uno, Las Pinas City, Philippines, after
having been duly sworn to in accordance with law hereby depose and state
THAT:
1. I am the legitimate father of JUSTINE KHYLE G. ROLDAN, who is
applying for his scholarship in Underwood Scholarship;
2. An Online Seller, with an earning of TEN THOUSAND PESOS
(10,000) monthly, respectively, I am a low income earner;
3. My annual income is so meager and insufficient for the needs of our
seven (7) children;.
4. I am executing this affidavit to attest to the veracity of the
above and in support to my son’s application to scholarship program.

having been duly sworn to in accordance with law hereby depose and state

1. I am the legitimate mother of JUSTINE KHYLE G. ROLDAN, who is
applying for  the his scholarship in We Underwood Scholarship
program;
2. I am not engaged in any business and profession.
3. I am executing this Affidavit to attest that as a plain house wife, I have
no income;
4. I am executing this affidavit to attest to the veracity of the
above and in support to my daughter’s application for a renewal to
scholarship program.

having been duly sworn to in accordance with law hereby depose and state
THAT:
1. I am the legitimate mother of JUSTINE KHYLE G. ROLDAN who is
applying for  the renewal of her scholarship in Underwood Scholarship
program;
2. That my Son, is a incoming 1st year college, taking Bachelor of
Science in Business Administration at Philippine Women’s University.
3. I am executing this Affidavit to attest that we have no registered
property and my daughter is not receiving financial or educational
assistance from other organization.
4. I am executing this affidavit to attest to the veracity of the
above and in support to my daughter’s application for a renewal to
scholarship program.
