AFFIDAVIT

I, Parveen Akhter, W/O. Late Md. Mehadi Hasan Sarker, of Vill. &
P.O. - Rajachapitala, P.S. Murad Nagar, Dist. Comilla, Bangladesh, by
Religion: Islam, by Profession: Housewife, by Nationality: Bangladeshi by
birth, age: 24 years, do hereby solemnly affirm and declare as follows:-

1. That, my husband Late Md. Mehadi Hasan Sarker, Passport # R

0624899 was working under Dallah Holding Co. in Makka Cleaning
Project, and died on 07/11/2007 naturally.

2. That I and other relatives of Late Md. Mehadi Hasan Sarker,

expressed our desire to get the dead body of Late Md. Mehadi Hasan
Sarker in Bangladesh and bury the dead body in our local graveyard.
The respected authority in Saudi Arabia is hereby requested to please
send the dead body to us in Bangladesh accordingly with an
intimation to me.
