nationality Bangladeshi, do hereby solemnly affirm and declare as follows:-
1. That I am a permanent citizen & National of Bangladesh by birth.
2. That I am sister of MD. SIDDIQUR RAHMAN of 60/2/A, West Madertek, P.O.
Bashabo, P.S. Sabujbagh, Dhaka-1214.
3. That my Niece named "SADIA UMMUL ADAN" was born on January 10, 1983 at
Dhaka, Bangladesh.
4. That MD. SIDDIQUR RAHMAN is Father and DELWARA BEGUM is Mother of
“SADIA UMMUL ADAN”.
5. That I was present at the time of birth of “SADIA UMMUL ADAN” on January 10,
1983 at 60/2/A, West Madertek, P.O. Bashabo, P.S. Sabujbagh, Dhaka-1214.
6. That as Aunt I am fully aware of correctness of the birth and relationship of my
above mentioned Niece.
