at Vill- Belki, P.O- Khunkhunia, P.S.-Sabang, Dist-
Paschim Medinipur, aged about 36 years, by caste-Hindu,
by profession – Daily Labour, do hereby solemnly affirm &
declare as follows,

1. That my name is Sri Chandan Shit which has been

correctly recorded in all my school documents including
in the certificate issued by the West Bengal Board of
Secondary Education in Madhyamik Examination and
also in myAadhaar Card vide No-729782913570 and also
in my Voter Card vide No-HBY1924042

recorded as Chandan Kumar Shit in place of Chandan
Shit in my Schedule Caste Certificate issued by Sub-
Division Officer,Kharagpur.

Chandan Shit ,S/O-Sri Bhaku Charan Shit and Chandan
Kumar Shit, S/O-Sri Bhaku Charan Shit all are the same
and identical person.
