By this deed I, the undersigned Ajay Bhardwaj previously called Ajay Kumar, doing store person and
resident of, Colyton, 2760 solemnly declare:

Reasons for not updating your new name on the Indian Passport:

In Australia, I was facing problems with my name as I had only my first name and no surname and
whenever I had to fill out any application or online form, I had to undergo so much trouble.

I was facing problems in Australia without a surname and I had no problems in India which is why I
never thought to add a surname while I was holding India passport.

Conclusion with general declaration taking responsibility for the above content

I, Ajay Bhardwaj, declare that the foregoing statements are true and correct to the best of my
knowledge and belief. I take full responsibility for the content of this affidavit and understand that
any false statements may result in legal consequences.

That for the purpose of evidencing such my determination declare that I shall at all times hereafter in
all records, deeds and writings and in all proceedings, dealings and transactions, private as well as
upon all occasions whatsoever, use and sign the name of Ajay Bhardwaj as my name/surname in
place and in substitution of my former name/surname.

That I expressly authorize and request all persons in general and relatives and friends in particular, at
all times hereafter to designate and address me, my wife, and remitter issue by such assumed
name/surname of Ajay Bhardwaj

Furthermore, I solemnly affirm that I am aware of the importance of keeping my personal

documentation accurate and up to date.
