AFFIDAVIT

I, KHALIDA BEGUM BARBHUIYA (formerly KHALIDA BEGUM LASKAR) D/O Faizul

Bari Laskar age about 34 years wife of Tapazul Ali Barbhuiya, resident of
Barbhuiya Lane, Ghaniala E&D Road, Silchar-2, PS: Silchar, Distt: Cachar (Assam),
by faith: Islam, by profession: Service, do hereby solemnly declare on oath and
state as follows:

1. That, I am a permanent resident and national of India by birth and

residing at the address given above.

2. That, I am married to Tapazul Ali Barbhuiya of the aforesaid address on

12th February 2005 as per the Islamic rites and rituals, both being persons
of the Mohammmedan religion.

3. That, since my marriage with said Tapazul Ali Barbhuiya I have been
living with him as his wife at his aforesaid place of residence.

4. That, since after my marriage with said Tapazul Ali Barbhuiya, I have had
adopted the surname “BARBHUIYA” with my name as per custom and
now known as KHALIDA BEGUM BARBHUIYA.

5. That, this affidavit is sworn for the purpose of declaring that henceforth I
shall be known only as KHALIDA BEGUM BARBHUIYA instead of
KHALIDA BEGUM LASKAR for all intents and purposes, and as such, in
my academic certificates, testimonials, appointment letter etc and
wherever my name appears as KHALIDA BEGUM LASKAR shall be read
and amended as KHALIDA BEGUM BARBHUIYA.

6. That, the averments made above are true to the best of my information
and knowledge and I swear and sign this affidavit before the Notary at
Silchar on this 24th the day of January 2022.
