Dear Sir,

With regards, and in exercise of the rights as enshirned under the RTI Act, I wish to seek from you
information as mentioned below:

1. Details of bank accounts including - current account, savings account, joint account,lockers,fixed
deposits and recurring deposits (hereinafter referrred to as ‘bank accounts’) in the name of Late
Shri _____ S/o _____, born on ______ resident ______.

2. Date of closure of the bank accounts

3. Certified copies of the statements of the bank accounts for 5 financial years till the date of
closure of such bank accounts.

4. Detail of nominee of the aforesaid bank accounts.

I solemnly declare that, I ______ am, a citizen of India (self attested copy of Aadhar card is enclosed
as annexure 1) and legal heir of ______ (self attested copy of Passport is enclosed as annexure 2).

It may be noted by your goodself that relying on the decisions of the Central Information
Commission (‘CIC’) in CIC/SBIND/A/2017/180954 dated 07.02.2018 and
CIC/VS/A/2013/000134/05484 (enclosed as annexure 3) ,the information requested for as above is
not covered under the ambit of section 8(1)(e) and 8(1)(j) of the RTI Act and therefore not capable of
being denied

May I request you to provide the requested information at your earnest. Any denial of provision of
information may be made after providing me a suitable oppurtunity of being heard.
