solemnly affirm and declare on oath as Follows :

A. That I have changed my NAME from JYOTI TO JYOTI NARENDRA TAYADE due to avoid any
future Controversy.
B. That I am producing this affidavit for the purpose of evidence for my NAME.
C. That both the NAME’S from JYOTI AND JYOTI NARENDRA TAYADE pertain to one and the
same person.
