AFFIDAVIT

I, SIBANI CHAKRABORTY, daughter of Late Bijay Bhusan

Chakraborty and wife of Late Subhas Chandra Chakraborty, aged
about 71 years, by faith Hindu, by occupation House-Wife, at
present residing at 8, Ramlal Bazar, P.O. Haltu and P.S. Garfa,
Kolkata 700078 do hereby solemnly affirm and declare as follows:

1. That in my Aadhar card being Aadhar No. 4428-9358-2167, as

well as in my PAN being PAN NO. ANBPC3034A where my date
of birth inadvertly written as on 08-01-1954.

2. That My husband Subhas Chakraborty @ Subhas Chandra

Chakraborty was died on 30-01-2024 and his death certificate Vide
 : 2 :

certificate no. D/2024/0067742 issued by department of health and

family welfare, Government of West Bengal, Kolkata Municipal
Corporation.

3. That I am Nominee in my decesed huusband’s pension book vide

P.P.O. No. R-10872 which was issued by Central Bank of India,
Central office, Mumbai-400021 and my date of birth appered in
pention book as on 07-01-1953.

4. That by swearing this affidavit I am declaring as my correct date of

Birth is 07-01-1953 as appered in my decesed husband’s pension
book.

5. That I am Indian citizen.

That the statements made above are true to the best of my knowledge
and belief.
