2. That my correct date of birth is HUSBAND DATE OF BIRTH and the time of
marriage; I was HUSBAND AGE at marriage time years old.
3. That I was unmarried till the time of marriage on 13/02/2004, I did not have
any other living spouse at the time of marriage.
4. That I fulfil all the terms and conditions of section 5 of Hindu Marriage
Act 1955.
5. That at the time of marriage I was not related to WIFE NAME Daughter of
WIFE FATHER NAME with the prohibited degree of relationship as per Hindu
Marriage Act.
6. That I belong to “Hindu Religion”.
7. That I am the Citizen of India.
8. That I declare that I was capable of giving valid consent for the marriage and of
sound mind not suffering from any mental disorder/insanity.
