and P.S. Kalighat, Kolkata 700026 do hereby solemnly affirm and
declare as follows:

date of birth written as 15-09-1966.

2. That in Hospital record which was issued by Ramkrishna Mission

Seva Pratishthan, my date of birth written as 15-09-1958 .
