CHANGE OF NAME AFFIDAVIT

I, Ms. Rekha Kumari Vadra, W/o. Vadra, Aged about 40 Years, residing at No.1,
Crescent Road, New Delhi, do hereby solemnly affirm and declare as under:-
1. That my name has been recorded as Ms. Rekha Kumari in all the
educational certificates and in other relevant documents etc.

2. That now after wedding I have changed my name as Rekha Kumari Vadra in
place of my previous name Rekha Kumari.

3. That by virtue of this affidavit, I changed my name from Rekha Kumari to

Rekha Kumari Vadra, in future I will be known by my new name i.e Rekha
Kumari Vadra in future for all purposes.

4. That the facts stated above are true to the best of my knowledge and belief.
