AFFIDAVIT
I, Kirti Azad, son of Late Bhagwat Jha Azad, aged about 64
years, by faith Hindu, by Occupation Social Worker, residing at
83A, Central Avenue Sainik Farm , Delhi Pin:- 110062, do hereby
solemnly affirm and declare as follows: -

1. That I am a Citizen of India;

2. That my name and surname is Kirti Azad, son of Late

Bhagwat Jha Azad and my said name and surname has been
recorded in my Voter Card or Elector Photo Identity Card
(EPIC) issued by the Election Commission of India, vide EPIC
No. SJE0483529 and in my Electoral Roll of 48 – Ambedkar
Nagar in NCT Delhi vide Serial No. 493;

3. That inadvertently and unwillingly my name “Kirti Azad” son of Late
Bhagwat Jha Azad has been wrongly mentioned in my Aadhaar Card
being No. 8789 8227 3723 as “Kirti Vardhan Azad” son of Late
Bhagwat Jha Azad and also in some of my necessary and official
documents or testimonials;

4. That “Kirti Azad” son of Late Bhagwat Jha Azad and “Kirti Vardhan
Azad” son of Late Bhagwat Jha Azad is the same and one identical
person;

That the Statement made above are true to the best of my knowledge
and belief I sign and swear this affidavit on this day the 05 th day of
April, 2024 at Durgapur Court.
