I, SYED JAHFAR SADIQ S/o. Mr. SYED MOHAMED, aged about 20

years, residing at No.23/12, 18th West cross street, M.KB. Nagar, Vyasarpadi,
Chennai-600 039, do hereby solemnly affirm and sincerely state as follows:

 I am the deponent herein.

 I state that I am an Indian citizen.
 I am residing at the above said address.

 I state that my father late Mr. MOHAMED ALI JINNA was expired on
06/09/2022, at GOVT. ROYAPETTAH HOSPITAL PETERS ROAD
ROYAPETTAH CHENNAI-600014, and it was registered by concern
authority, Issued by Department of Public Health, Greater Chennai
Corporation, Vide Registration No. COC/2010/07/111/001405/0.
 I state that my Grand Father Late Mr. IBRAHIM already expired before
forty years but it was not registered by concern authority due to
unawareness.
 I state that my name was wrongly mentioned in my father’s death certificate
instead of the name of my Grand Father’s (IBRAHIM).
 I state that my Grand Father’s name was not mentioned in my father death
certificate.
 So, Kindly enter the my grandfather name Late. Mr. IBRAHIM, in my
father’s death certificate.
 I swear this affidavit to be produced before concerned authority
whomsoever.
 I submit that all the facts stated above are true and correct to the best of my
knowledge and belief and nothing material has been concealed there from.
