AFFIDAVIT OF DISCREPANCY

I, RIZALDY RODRIGUEZ, of legal age, single, Filipino and a resident of

Daungan Ilaya, Catanauan, Quezon, Philippines, after having been duly sworn to in
accordance with law hereby depose and state that:

1. I am an illegitimate son of Patricio Sanchez who was born on 30 December 1996;

2. On 20 January 1997, my father, Patricio Sanchez executed an affidavit of

acknowledgment/ admission of paternity attesting that he was my father;

3. Based on such affidavit of paternity, I used the surname of my father, Sanchez in

school and in all my documents, transactions, and dealings;

4. However, when I secured a Certificate of No Marriage in the Local Civil Registrar,

I was informed that I cannot use the surname of my father and that I can only
use the surname of my father;

5. My names: Rizaldy Rodriguez as appearing in my Certificate of Live Birth and

Rizaldy Sanchez as used in all my identification cards and school records refer to
me: the same person; and

6. I am executing this Affidavit in good faith to attest to the truth of the foregoing
facts and for all legal intents and purpose this may serve.
