AFFIDAVIT OF DISCREPANCY

I, RICA JAVIER REPRADO, of legal age, Filipino, single, and residing at

No. 2638 Lico Street, Tondo, Manila, after having been duly sworn to in
accordance with law, hereby depose and state:

1. That I was born on August 20, 1956 in Paco, Manila and my parents
are CRISTINA JAVIER RAMOS (deceased) and GEDEON LATCHICA
REPRADO (deceased);

2. That in my Certificate of Live Birth issued by the Philippine Statistics

Authority under Local Civil Registrar No. 4319 (R56), my name was
recorded as ERICA JAVIER REPRADO;

3. That RICA JAVIER REPRADO is the name known by my relatives and

friends and this is the name appearing in my school records and other
pertinent legal documents bearing my signature;

4. That the name ERICA JAVIER REPRADO and the name RICA JAVIER
REPRADO refers to one and the same person;

5. That I am executing this affidavit to attest to the truth of the foregoing

facts and circumstances and for whatever all legal intents and purposes
this instrument may serve.
