AFFIDAVIT OF DISCREPANCY

I, CORAZON REPOL TAFALLA VILLAR, of legal age, Filipino, married,

and residing at No. 2138 V. Serrano Street, Tondo, Manila, after having been
duly sworn to in accordance with law, hereby depose and state:

1. That I was born on June 8, 1949 in Tarangnan, Samar and my parents

are GAUDENCIA DAROY REPOL (deceased) and GAUDENCIO
LABANGCO TAFALLA (deceased);

2. That in my Certificate of Live Birth duly registered at the National

Statistics Office under Registry No. 2009-088, my name was recorded
as CORAZON REPOL TAFALLA;

3. That RUBEN GABIJA VILLAR and I were married on July 27, 1977 in
CDCC BR Chapel located at No.2279 A. Arellano Street, Sta. Ana, Manila
and duly officiated by Rev. AQUILINO NAVARRO, Marriage Minister, as
duly evidenced by Marriage Contract Registry No. 930 (g-77) issued by
the National Statistics Authority;

4. That in my last SSS contribution, my name was recorded in the Social

Security System as CORAZON VILLAR instead of the correct name
CORAZON REPOL TAFALLA VILLAR;

5. That the erroneous name CORAZON VILLAR and the correct name
CORAZON REPOL TAFALLA VILLAR refer to one and the same person;

6. That I am executing this affidavit to attest to the truth of the foregoing

facts and circumstances and for whatever all legal intents and purposes
this instrument may serve.
