AFFIDAVIT

I Surendra Samal aged about 60 years, S/o Sana Khali Samal Residence of village-
Andirasingi, Po Kachakhandi, Ps Sheragada, Dist Ganjam, Odisha do hereby by solemnly affirm and
sincerely state on oath as follows.

1. That, I am the deponent of this affidavit.

2. That the ROR of mouza Andirasingi vide Khata no. 507, of total extent area 3.5180 Dml has been
recorded in my father’s name namely Juria Bisoyi S/o dulabha Bisoyi.
3. That, I am enjoying the above mentioned property by paying land revenue to the
Govt. regularly.
4. That, at present at present mu son Srikant Samal S/o Surendra Samal ha applied for
Cultivation of Hybrid Vegetable under OIIPCRA in the above mentioned land out of
1.500 Dml out of 3.5180 Dml (Plot Nos. 207, 2072, 2371, 2190, 1633, 1787)
5. That, I swear this affidavit to be filed before the concerned authority of Horticulture
Dept., ADH, Bhanjanagar for his/her satisfaction and do the needful.
6. That the facts stated above all are true to the best of my knowledge and belief.
