AFFIDAVIT

I, Suchitra Panigrahy, aged above 39 year, D/O. Prabhakar sahu, Resident of village- Belagaon, PO-
Bhatakumarda, PS- Dist. Ganjam, Odisha, do hereby solemnly affirm and state on
Oath as follows:-

1. That, I am the deponent of this Affidavit.

2. That, the landed property situated in Mouza- Belagaon in Khata No. 809/1430, Plot No.
4420, measuring an extent of Ac. 0.031 Dec., under Purushottampur Tahasil stands recorded
in my name.
3. That my Nayana kumar Panigrahi intendes to take on rent basis for above mentioned
agriculture land for the purpose of cultivation.
4. That my Nayana kumar Panigrahi has applied for agriculture loan in the above mentioned
lands before the Authority.
5. That I have NO OBJECTION if the authority will sanctioned the subsidy loan amount in favour
of my Nayana kumar Panigrahi.
6. That I swear this Affidavit before the Notary Public, Berhampur for its production before the
Appropriate Authority for subsidy purpose.
