I, Santosa Kumar Gantayat, aged about 31 year S/O. Surendra Gantayat ,Brahmin by caste, resident
At- VIP Colony, Kamapalli, PS- B.N. Pur, Berhampur, Odisha. Do hereby declare that:

1. That I am the deponent of this affidavit.

2. That, I have applied housing loan under Pradhan Mantri Awas Yojana- Credit Linked Subsidy
Scheme.
3. That I submit the declare that I do not own a pucca house ( an all-weather dwelling unit)
either in my name or in the name of any member of my family any part of India.
4. I submit and declare that my family has not availed of central assistance under any housing
scheme from Government of India.
5. I have applied for housing loan under the subject scheme and the carpet area of House/ Flat
to the purchased/ constructed is less than 30 Sqm/60Sqm/160Sqm/200Sqm
That content in the above said affidavit are true and correct to the best of my knowledge
and belief.
