After 6 hours (attached Screen shot on 11:08PM) water leak at hot line noticed in 10th floor. The leak occurred due to crack/damage in thermo-mixing valve compression fitting. Description of Consequences {Including Injury/linesses): In addition to the damage in ceiling, the following MEP items had been damages: Exit light Public address speaker (2 nos) Fire alarm Detector Evacuation speaker Immediate Actions Taken: 1) Leak had been rectified - compression fittings replaced. 2) Domestic water System had been energized again, 3)_24/7 watchman kept at site to monitor and stop immediately any leak _ ‘Attachments: 4) Copy of Work Permit 5) CCTV Screen Shoot 6)_ Photo for Thermo-mixing valve.
