SUBJECT : Incident Report re: Accidental Firing Incident Inside


Camp Karingal Involving DPSB-SRU Personnel

DATE : February 17, 2017

1. References:
a. DHSU Logbook dated February 17, 2017 with title Incident Report re:
Accidental Firing; and
b. BJMP Blotter Book with Entry Number 0776 dated February 17, 2017.

2. Above references pertains to an alleged accidental firing incident that


transpired on or about 7:45PM of this date involving a member of the Special Reaction
Unit of the District Public Safety Battalion, QCPD.

3. Initial investigation conducted by this Unit, personnel of the Quezon City


Female Dormitory, BJMP were startled by the sound of a gunshot coming from the
DPSB Personnel Barracks, Camp Karingal, Sikatuna Village, Quezon City. Upon
inquiry of JSUPT MARIE ROSE LAGUYO, Jailwarden, QCFD, BJMP it was learned that
a victim was an inmate who was identified as a certain MARY GRAVE LASING @JANE
who was injured by a stray bullet coming from the direction of DPSB Barracks.
Immediately, the victim was rushed to Quirino Memorial Medical Center driven by JO1
Joseph Raymond Comia and escorted by SJO2 Fe Galve, Duty Nurse, QCFD, BJMP.
Fortunately the victim only suffered superficial laceration on her right temple and now
undergoing treatment.

4. Further inquiry made by the undersigned shows that the QCPD personnel
involved in the said accidental firing incident was identified as PO1 ERICSON ESLAO y
SACRATEN, 30 years old, PNP member assigned at the Special Reaction Unit (SRU)-
District Public Safety Battalion and resident of No. 9 Caldozo St., Purok 6-B Lower
Bicutan, Taguig City. It appears that on the said time and date, while PO1 Eslao was
unloading his issued 5.56 Cal. NORINCO Armalite Rifle with serial number 001709
inside the DPSB Barracks when it accidentally fired to the direction of the QCFD
Female Dormitory.

5. The QCPD personnel is now under the custody of PCINSP MONSALVE of


Criminal Investigation and Detection Unit and the case is currently being investigated by
PO3 Joselito C Gagaza and proper disposition by the said unit is being undertaken.
