Congratulations!

We are very pleased to welcome you to Saint Louis University!

Here are the procedures on the IN-PERSON ENROLMENT for Academic Year 2022-2023 for
your information and guidance. The IN-PERSON ENROLMENT PROCEDURE is intended for
the QUALIFIED FRESHMEN APPLICANTS. Please strictly follow the ENROLMENT DATE
assigned to you.

IMPORTANT: Please READ the COMPLETE steps BEFORE COMING to SLU for your
ENROLMENT. In addition, any misrepresentation /concealment of information will be
enough reason to disqualify/invalidate your enrolment in SLU.

For QUALIFIED BS in Nursing Applicants: Apart from the Basic Requirements required for all
Qualifiers, BSN Qualifiers will have to download, fill-up and prepare the following:
(1) Scanned copy of the duly signed School of Nursing Undertaking*
(2) Scanned copy of the medical certificate with complete laboratory results or a medical
certificate WAIVER*, and
(3) Photocopy of your Form 138. PLEASE WAIT FOR AN EMAIL CONFIRMATION BEFORE
PROCEEDING WITH YOUR IN-PERSON ENROLMENT. The forms can be downloaded from
your iSLU Portal , “Downloadable” Tab.

Steps for the IN-PERSON ENROLMENT of QUALIFIED FRESHMEN Applicants


for AY 2022-2023

(Bishop Carlito Cenzon Sports Center formerly SLU Covered Courts, SLU Gate 2, Bonifacio St.)

Before your scheduled enrolment, please ensure to prepare the following:

a. SHS Grade 12 Report Card (if not yet available, Certificate of


Graduation that contains date of release of your Report Card signed by
your SHS Principal);
b. Photocopy of Birth Certificate (PSA or NSO Copy);
c. Certificate of Good Moral Character;
d. Random Drug Testing Acknowledgment Form* (Random Drug Testing
is NOT required, kindly fill out the form
e. 2 (Two) 2X2 ID Pictures with Name Tag and White Background;
f. OSA Form*
g. SLU Data Privacy Policy Consent Form*
h. Promissory Note* (If any of a, b, and c is/are not available yet)
i. Certification for Top 1 or Top 2* for students who belong to the Top 1
or 2 of their Strand/Batch. The appropriate Tuition Fee discount will be
applied to your individual account for the First Semester, AY 2022-2023
after you have been officially enrolled. Please submit the hard copy of
your Certificate at Window 11 of the Finance Office from August 15 – 20,
2022.
j. For Persons with Disability – Download the Student with Special
Needs Form* to be submitted together with the other documents. In
addition, a Medical Certificate stating that the applicant is “FIT to
ENROLL” must be submitted or e-mailed to the Guidance Center at
gcdir@slu.edu.ph within the first three (3) weeks of classes. An
applicant who fails to submit the required certification from his/her
attending physician may be asked to withdraw with permission as the
main intention is to ensure his/her well-being or best interest.
k. Photocopy of at least one Government issued ID with photo of your
parents OR legal guardian in Baguio City, with their original signature
affixed beside each photocopied signature to correspondingly
authenticate.
l. For QUALIFIED International Students: For International students,
who are in the Philippines, they will need to submit the additional
requirements (1) Photocopy of Valid Visa, (2) Photocopy of Passport
Bio Page, (3) Recommendation Letter, and (4) NBI Clearance. For those
who are abroad/or in their home countries, there is no need to submit
the additional documents

During your scheduled enrolment date:

1. Triage Process. Proceed to the triage area at the entrance gate of the Bishop Carlito
Cenzon Sports Center and present your completely filled-up Health Declaration Form
and Vaccination Card/Certificate. Only fully vaccinated students (Full 2 doses with
booster) will be allowed to enter the enrolment venue. If accompanied by a
parent/guardain, only 1 parent/guardian will be allowed subject to the submission of
the HDF and Vaccination Card (received 2 doses with booster).

2. Enrolment/Admission Proper. After the triage process, proceed to your respective


admission/enrollment booth inside the enrollment venue;
a. Submit your duly accomplished SLU-OSA FORM, CREDENTIALS and OTHER
ENROLLMENT REQUIREMENTS except down payment to the Admission Officer;
b. Admission Officer Fills up the PERMIT TO ENROLL, Checks the submitted
requirements and Encodes your Student Information and Schedule;
c. Get your printed ENROLLMENT FORM. Read carefully and sign at the front and
back of the form.

3. Payment of Fees:
3.a Proceed to the Finance Office at the 2nd Floor Lobby, Diego Silang Building for
the:
I. Encoding of SLU ID Number (Window 6 or 7)
II. Payment of Corresponding Fees (Windows 9 to 12 )

3.b For International students, proceed to Window 7 of the University Registrar’s


Office for appropriate clearance before proceeding to the Finance Office.

3.c For SLU Employee Dependents proceed to the HRD Booth at the Enrollment Area
before proceeding to the Finance Office.
4. ID Processing. After payment of fees, proceed to the PE Building (near Enrolment
Hall) for Photo taking for Student ID.
5. Parent Portal. Parents/Guardians can register for an online access for the monitoring
of their child/ ward’s records at the UNIVERSITY REGISTRAR’S OFFICE (WINDOW 10,
Ground Floor Area.)

6. Claiming and Release of SLU IDs – Claiming of Student IDs will be on August 15 – 20,
2022 at Windows 5 and 6 of the University Registrar’s Office, Diego Silang Building.
