JOINT AFFIDAVIT
We, GERAMIE VILLAVICENCIO and EDITHA GABONADA, both of legal ages,
Filipinos, and residents of Iligan City, after having duly sworn to an oath as required by
law, hereby depose and say:

1) That we personally know the person of PERLA LAGATA DAHILOG, born to

Antonio Parcotan Dahilog and Fernanda Jumawan Lagata on April 14, 1964;

2) That said PERLA LAGATA DAHILOG was legally married to Joselito Pepito
Dingcong March 19, 2007 in Iligan City as evidenced by a Certificate of
Marriage issued by the Local Civil Registrar Office of Iligan City hereto
attached forming part hereof;

3) That in the said Certificate of Marriage, however, the middle name of the wife
PERLA LAGATA DAHILOG has been reflected as “LABATA”;

4) That the correct middle name of the herein mentioned person of “LAGATA”,
and the names either be, PERLA LAGATA DAHILOG or PERLA LABATA
DAHILOG, refer to one and the same person of PERLA DAHILOG
DINGCONG as married person;

5) That we know the foregoing facts as we are long time friends and neighbors
of the said PERLA LAGATA DAHILOG, now PERLA DAHILOG
DINGCONG;

6) That we are executing this affidavit to attest to the above facts and for such
other legal purpose it may serve best.
