I am writing to request for an original copy of the documents listed below in

relation to a criminal case filed against me before the Office of the Provincial Prosecutor
of Leyte, docketed as NPS No. VIII-02-17K-00293 for Grave Oral Defamation, to wit:

1. Judgment/Decision/Order/Resolution, whichever is applicable;

2. Entry of Judgment/Certificate of Finality, whichever is applicable; and,
3. Clearance from the court, prosecutor’s office or agency concerned.

The documents I am requesting are directly related to me as I am the respondent

in the abovementioned case.

I am requesting for an original copy of the documents enumerated above as

these are essential requirements which need to be attached as annexes to my Petition
to Take the Bar Examinations pursuant to a Resolution issued by the Supreme Court en
banc dated March 9, 2021 (Re: B.M. No. 3490).
