TO WHOM THIS MAY CONCERN:

I am NOT REQUIRED to attach any document in this field. The case filed against
me was decided before the Provincial Prosecutor’s Office of Leyte and did not reach the
court. This requirement (Certificate of Finality of the Decision) is not applicable to me. I
am only uploading this letter because I cannot proceed with the re-submission of the
petition documents which were returned to me for correction if this field is left empty.

A prompt which says “Please upload required documents” appears on the

screen when I click the “SAVE” button, even when the upload of the required
documents is already completed. I was advised by the Office of the Bar Confidant to just
attach a letter on the Pending/Decided Cases field even when upload of documents in
this field is optional because this might be the reason I cannot save my upload. This in
the meantime, since waiting for the response from the service provider may take a
while.

I have attached in this letter screenshots of what appears on the screen when I
click the “SAVE” button for your scrutiny.
