AFFIDAVIT OF CONSENT

I, EMIL L. BERNARDINO, of legal age, married, Filipino and resident of

Barihan, Malolos City, Bulacan, after having been duly sworn to in accordance with law
do hereby depose and say:

1. That I am the biological father of SEAN MCKENZIE F. BERNARDINO,

17 years of age, born on February 22, 2002 in Malolos City, Bulacan;

2. That my son is applying for a Student License with the Land Transportation
Office (LTO);

3. That taking into consideration that my son is a minor, I, being his father and
legal guardian, I hereby give my full consent for him to apply/ renew a
Student License from the Land Transportation Office;
4. That I am executing this affidavit to attest to the veracity of the facts
contained herein and for requesting the LTO to issue a license for my son
SEAN MICKENZIE F. BERNARDINO and for whatever legal intents
and purposes it may serve.

IN WITNESS WHEREOF, I have hereunto set my hand this 13 th day of March,
