I am writing this letter in connection with the incurred deductions I had from my salary. I am
KELLY BOY B. FERNANDEZ, a driver assigned at JRMT-ILIGAN HUB since September
1, 2021. This letter is to respectfully request your good office not to deduct from my salary the
following contributions: SSS, PhilHealth and PAGIBIG since I am already a self-employed
member of these government institutions.

In connection, I would like to request for a refund of these deducted contributions from my
previous salary dated last September 1-15, 2021 payroll amounting to P 702.00 (SSS: P 427.00,
PhilHealth: P 175.00 and PAGIBIG: P 100.00).

I am looking forward for your favorable response regarding this matter.
