Your son, Jaylon Miguel has been constantly distracted in Integrated Science Class. Majority of the class
time Jaylon spends with his head down on the table and doesn’t move until the teacher asks him to
(more than once). Jaylon dislike writing down notes or contributing positively to the class.
Whenever I (teacher) have to call on Jaylon’s attention it interferes with his learning and that of the
entire class. I have spoken to Jaylon about his attitude and behavior during my class time however, there
has been little to no improvement to his behavior.

Last week the behavior escalated to an unwanted level as Jaylon had joined along with another
classmate to disrupt class by being sarcastic and answering back the Teacher. As stated in the handbook,
consequences for such actions, teacher asked Jaylon, along with the next student to report to the vice-
principal’s office until after the class session was done.

Jaylon did not report to the vice-principal and until teacher went to see the vice-principal he reached the
office with a big grin on the face. As punishment for their unruly behavior, Jaylon was asked to perfom
community service for the Environmental Club. Jaylon has completed the task but the task was not
completed correctly & therefore he will be asked to do it over.

Due to these incidences, Jaylon will be continuously monitored over the next few days and a report will
be sent to the Vice-Principal’s office at the end of the week. If his behavior does not improve by then,
the next step to be taken will be a parent-teacher conference to discuss this matter & its consequences.
