INFORMED CONSENT FORM

This informed consent form is for police personnel I am inviting to participate in


research, entitled “Lived Experiences of Chief Investigators of the Different Cebu
Police Stations in the Cities of Central Visayas During the COVID-19 Pandemic”.

Part 1: Information Sheet

Introduction

I am a student researcher from the University of Cebu, Graduate School.


This research pertains to “Lived Experiences of Chief Investigators in the Cities of
Central Visayas During the COVID-19 Pandemic”. I am going to give you
information and invite you to be part of this research. You do not have to decide
today whether or not you will participate in the research. Before you decide, you
can talk to anyone you feel comfortable with about the research.

This consent form may contain words that you do not understand. Please ask
me to stop as I go through the information and I will take time to explain. If you
have questions later, you can ask me or to other researcher.

Purpose of the Research

To know the lived experiences of chief investigators of the City Police


Stations in Central Visayas during the COVID-19 pandemic as well as the
perceived solutions on negative experiences of the chief investigators relative to
community crimes amid the COVID-19 pandemic.

Type of Research Intervention

This research will involve your participation in an interview that will take
only a few minutes.
Participant Selection
You are being invited to take part in this research because I feel that your
views as Chief Investigators of the City Police Stations and in the City and
Provincial Police Offices in Central Visayas can contribute much in my
understanding and knowledge to this study.

Voluntary participation

Your participation in this research is entirely voluntary. It is your choice


whether to participate or not. If you choose not to participate, it would be okay.

Procedures

I am going to ask you to share with me with your experiences during the tour
of your duty and being assigned in one of the city police stations and in City and
Provincial Police Offices in Central Visayas. If you allow me, then you will be
asked a question relative to your assignment as chief investigators.

If you do not wish to answer any of the questions during the interview, then
you may pause and inform the interviewer so we can move to the next question.
The information recorded is confidential, and no else will access to the information
documented during the interview. The entire interview will be tape-recorded, but
no one will be identified by the name on the tape. The tape will be kept in the
Research Office of the University of Cebu. The information recorded is
confidential, and no one else except the Research Director will have access to the
tapes and the said tapes will be destroyed after a week.

Duration

The research takes place almost one year in total. Within that duration, I will
visit you for two to three times for a follow up interview.

Risks

If the discussion is on sensitive and personal issues, and you may feel
uncomfortable talking about some of the topics, you do not have to answer any
question or take part in the interview. If you don’t wish to do so, it would be okay
with me. You do not have to give me any reason for not responding to any
question, or for refusing to take part in the interview.

Benefits
To help improve the performance of chief investigators assigned in the Cebu
City Police Office and help transform the negative to positive experiences in field
duty.

Reimbursements

You will not be provided any incentive to take part in the research.

Confidentiality

The research being done may draw attention by other people in the PNP
organization. I will not share any information about you to anyone outside of the
Panel of Examiners. The information that I gathered from this research project will
be kept private and given utmost confidentiality. Any information about you will
have a number on it instead of your name, and only the researcher will know what
your number is and I will lock that information up with a lock and key. It will not
share with or be given to anyone.

Part II: Certificate of Consent

I have been invited to participate in research about the Lived Experiences of


Chief Investigators of the city police stations and in the City and Provincial Police
Offices in Central Visayas During the COVID-19 Pandemic. I have read the
foregoing information, or it has been read to me. I have the opportunity to ask
questions about it and any question. I voluntarily submitted myself to be a
participant in this study.
