I M.Siva Prasad Reddy give my exclusive consent for the position of Material Engineer or any
suitable position to Theme Engineering Services Pvt. Ltd. Jaipur for the following projects:

Selection of Consulting Services for: Construction Supervision Consultancy Services for

Supervising the CKICP Road Upgradation Work Contracts (Package-CSC-03) Project: Chennai
Kanyakumari Industrial Corridor Project

I further confirm that I will not give my CV or consent to any other Company / Body / Corporation for any
other project till completion of the above projects.

My all inclusive remuneration will be Rs.35, 000 (Total Rupees Thirty five Thousand only) per Month
which shall be paid on monthly basis as per actual presence at site. I will be available to join this
project, as and when asked to join the project site.

I undertake that:

 I have neither been black – listed / debarred by any Client / Employer, nor have I been
dismissed from service (by any of my previous employers) on moral or ethical grounds.

 I have never abandoned any Design / Supervision assignment of any of my previous employer
before its scheduled completion.

 Copy of certificates towards proof of age, qualification and experience are enclosed for your
perusal, and I vouch for their authenticity.

 I do not have physical deformity / handicap. I am in good health and fitness and will be able to
perform my services for this project to your satisfaction.

 I am aware of the fact that client shall not accept any replacement till the assignment is
completed and therefore I confirm that I shall be available for the entire duration of the project
and this assignment.
