I hope this letter finds you well. This is to inform your good office regarding the
reconciliation of the FORM 6 (LEAVE FORM) submitted in your good office effective
June 6, 2022 to September 21, 2022, Maternity Leave of Absence with Pay.
However, my approved leave of absence from the ONLINE LEAVE SYSTEM was
dated July 15, 2022 to September 22, 2022, which indicated a long gap to the
originally filed FORM 6.
Anent to this, I would wish to reconcile the file so I could be reinstated on
September 22, 2022.
