AFFIDAVIT OF UNDERTAKING

I, AIZA ESCOL CATAYAS, of legal age, married, Filipino, and a resident of

Regency Plains Subdivision, Phase Block 26, Lot 11, Gumamela Street, Brgy. Iponan,
Cagayan de Oro City, after having been duly sworn to in accordance with law, do hereby
depose and say that:

1. I am presently married to EUROCHIC DELOSO CATAYAS to whom I have three

children;

2. At present, my relationship with my husband is on tension;

3. Given the rift between us spouses and to prevent the conflict to escalate, I will allow
my husband temporarily live away from the family home based on the above-
mentioned address. I and the children will retain the family home;

4. During that period, I will not interfere with the personal life and whereabouts of my
husband, EUROCHIC DELOSO CATAYAS unless he will neglect his
responsibilities towards our children and/or he contracts a subsequent marriage
without any Judicial Pronouncement of Nullity of our marriage in accordance with
the Family Code;

5. Despite the separation de facto, I will not neglect my obligations towards our
children which include but not limited to providing love, respect, and
support(emotional and financial);

6. Any violation of this undertaking shall result in the imposition of civil or criminal
liabilities including but not limited to Bigamy, Adultery, and all other relevant laws;

7. Notwithstanding, I will try my very best to reconcile with my husband;

8. I am executing this Affidavit to attest to the truth of the foregoing and for whatever
legal purpose this may serve.

IN WITNESS WHEREOF, I have hereunto set my hand this September 1, 2022 at
