AFFIDAVIT OF LATE REGISTRATION

OF AN ILLEGITIMATE CHILD

I, NILDA P. SIENDA, single, of legal age, Filipino and, presently residing

at Paclasan, Roxas, Oriental Mindoro, after having been duly sworn in
accordance with law, do hereby depose and say:

1. That I am the mother of JHON ALI SIENDA who was born on

January 15, 2016 at Sanchez-mira, Cagayan;

2. That at the time JHON ALI SIENDA was born, his natural father and I
were not married;

3. That due to inadvertence, the birth of my aforesaid son had not been
registered in the Municipality of Sanchez-mira, Cagayan or in any
government agency up to the present;

4. That I am executing this affidavit to attest to the above facts and to

confirm the illegitimate status of my granddaughter;

IN WITNESS WHEREOF, I have hereunto set my hand this 20 th day of

September 2021 at Roxas, Oriental Mindoro, Philippines.
