Here’s the list of requirements and timelines you need to meet to ensure completion.
PRE-ONBOARDING REQUIREMENTS | Required BEFORE start date!
Upload the following on your Onboarding profile:
1. SSS No:

5. Completely Filled Out BI Form
TYPE A REQUIREMENTS | Due: Fifteen (15) business days from start date
Upload clear copies of the ff documents on your Onboarding profile: (Note: Max 2mb size limit, JPG, PDF, PNG)
1. SSS Document
2. TIN Document or BIR 1902 Form ONLY if No TIN yet
3. PhilHealth Document
4. Pag-Ibig Document
5. Original NBI Clearance (valid three months from start date)
6. Completed Medical Slip
7. Fit to Work Clearance (only if applicable – please wait for confirmation from your ORBIT POC)
8. Birth Certificate (PSA/NSO)
9. Marriage Certificate and Dependent’s Birth Certificate (PSA/NSO) only if applicable
10. Other additional program requirements
11. LGU Requirement: Health Certificate & Occupational Permit (For MOA, Davao and Quezon City Hires only)

TYPE B REQUIREMENTS | Due: Thirty (30) business days from start date

BACKGROUND INVESTIGATION REQUIREMENTS (specific to the account you are assigned to)
Note: Your ORBIT POC will get in touch with you for your program’s background investigation requirements.
I know how much your NEW job in Concentrix matters to you so I have also attached soft copies of the following
documents for you to fill out and sign. Please submit upon acceptance of employment agreement.
Please download/ fillout and upload clear copies of the following documents on your Onboarding Profile:
1. Background Investigation Form
2. BI Undertaking Agreement and BI Checklist
3. BIR Form 2316 (if applicable)

Thinking you might have trouble printing and scanning your docs? We at Concentrix care about our environment so we prefer to go
paperless! Here’s how to use to a third-party e-signature website to sign your docs digitally:
1. Go to this link https://www.docsketch.com/online-signature/
2. Click on “Draw Signature”
3. Sign on the signature box using your mouse or touch pad. (Make sure that you choose color black)
4. Click “Save”
5. Click “Download”
6. Open the downloaded file
7. Right click, “Copy”
8. Paste the e-signature in the Background Investigation Form
9. Save the signed BI Form then send back to us by replying to this e-mail thread

Metrobank Payroll Account Enrollment Guidelines:
Here’s how you can enroll for your Metrobank Payroll Account. We’ve made it easier for you by letting you nominate the
nearest branch where you can claim your ATM card. Please ensure to complete the following steps ON YOUR START
DATE. Failure to do so would mean delays on your salary release.

Step 1: Click https://forms.office.com/r/7WufNGgSGb. It is important that you accurately fill out all the required
information. This step will initiate your payroll account creation.

Step 2: While waiting for your ATM card availability, you must email the following requirements to ensure salary
release.

Send the following to Metrobank’s email address: mbtcactivation@metrobank.com.ph (Note: PDF format required,
max of 10MB; Email Subject: CNX_[LastName, FirstName, MiddleName])
• Filled out Customer Information and Data Consent Form: https://www.datocms-
assets.com/26060/1606382509-customer-information-form-individual.pdf
• 3 specimen signatures on a clean white paper
• Valid acceptable ID with specimen signature
Take Note! Metrobank will place your account on hold until the requirements have been submitted to the Metrobank email provided.
You only have 15 days from date of Payroll credit to submit the requirements. Failure to do so will cause the closing of your account
which can only be reopened once you personally submit and claim your ATM card at your nominated branch.

Step 3: Once your ATM card is available for pick up, visit your nominated Metrobank Branch, to sign the account
opening forms and claim your ATM card and PIN mailer.

• You must bring at least 1 valid Government Issued ID (Original ID and Photocopy)
• Once ATM card has been claimed, you must request lift of hold-out by sending an SMS to 09191606282
CLAIMED<space>Last four (4) digits of ATM card<space>Employee ID Number<space>Date of Birth
(format in mmddyyyy) Example: CLAIMED 1234 101812345 05151990
• You will receive an SMS notification whether lifting is successful or not
If you are unable to pick up your ATM card but have accomplished Step 2, you may set up your Metrobank online account for cardless
transactions. Download the Metrobank Mobile Banking App and create your account. This step will let you use your Metrobank
account for online transactions while your ATM card is still in transit.

Take Note! Authorized representative will not be allowed to claim on behalf of the employee.

List of Acceptable IDs:


• NBI Clearance/ Passport/ Driver’s License /PRC ID
• Postal ID / Voter’s ID
• SSS / GSIS ID / UMID
• Senior Citizen Card / PWD ID
• TIN ID / Pag ibig ID (not the Loyalty card) / New Philhealth ID (with picture and signature)
• Overseas Filipino Worker ID / Seaman’s Book / Alien Certificate of Registration or Immigrant Certificate Registration
• Concentrix ID
