AFFIDAVIT OF SUPPORT

I, __________, of legal age and a resident of #_______________,

after having been sworn in accordance with law, hereby depose and state:

1. That I am a close friend of ____________;

2. That my afore-named friend and myself, will be traveling to

Madrid, Spain on the _____________;

3. That I undertake to shoulder Ms _________ round trip airfare for

the intended trip, together with the accommodation and all other costs of the
trip;

4. That I am financially able to support her for the said travel, I, being
employed as Chairman of ________________, a company established in the
_______________in 2013;

5. That I am executing this affidavit to attest to the truth of the

foregoing statements.

6. That I have purchased airline tickets with Emirates and have

attached a copy of the tickets with the Visa application.

7. In accordance with the requirements I have purchased travel

insurance for Ms __________ and a copy is included with the Visa
application.

8. In accordance with the requirements I have booked and paid for all
accommodation for the duration of the trip for Ms _______ and copies are
included with the Visa application.

9. In accordance with the requirements Ms _________ has justified

the purpose of the stay with the Visa application. As stated in the application
Ms _______________ is accompanying me to Spain to watch my daughter
compete in the World Duathalon Championship in Aviles. Ms
_______________ accompanied me to Australia last year to watch my
daughter compete in the 2015 World Duathalon Championship in Adelaide.

10. In accordance with the requirements Ms _______________ has

demonstrated sufficient financial means. This affidavit together with copies
of pre-paid tickets and accommodation as well as copies of my credit cards
and bank statements demonstrate that Ms _______________ has financial
means. Further, I am a well established business man with assets in Australia
and the Philippines. I travel regularly with Ms _______________ and
always travel according to legal requirements. Ms _______________ and I
reside in The Philippines which is our home.

11. I further state that the information is reliable as is attested by this

affidavit.

12. The pre-purchase of airline tickets, the past history of travel, the
reason for the trip, the need to return to the Philippines for business purposes
all attest to the intention of Ms _______________ and I returning long
before the expiry of the tourist visa.
