AFFIDAVIT TO RETURN HOME

I, _________________, of legal age, a Filipino citizen, single and presently residing at

________________________, after having been duly sworn to in accordance with law,
hereby depose and say:

1. That I was able to secure a job as a shellfish laborer in Fisherman’s Market

International Incorporated owned by ____________ with business address
located at _________________, attached herewith is my employment contract
as Annex “A”;

2. That the employment contract will commence once I resume my official functions
in the company of _____________;

3. Further, one of the conditions embodied in the employment contract is that I am

required to secure a valid work permit and/or visa to secure my successful entry
to Canada;

4. That when I applied for a visa on ______________ I was not granted because
my return to the Philippines after my employment contract is not guaranteed;

5. That my reason for securing a job in Canada is to earn money to support my plan
to go back to school and finish a course in Medicine, become a doctor, to serve
the less fortunate people in my country;

6. Besides, my parents are highly respected in our community and I will not do
anything to embarrass them and tarnish their reputation;

7. Lastly, being the eldest child of my parents, I am responsible to take care of them
in their old age as well as my younger brother who is still studying in our
hometown;
 8. I am executing this affidavit to show that I have no intention of staying in Canada
beyond the expiration of my work permit and/or visa and attest to the truth of the
foregoing facts.
