AFFIDAVIT OF ACKNOWLEDGMENT, AND

AUTHORIZATION TO DEDUCT

been duly sworn to law, do hereby depose and state, that:

1. I am currently employed by Pieceland Corp. as

_______________________________;

2. I acknowledge and truly understand that the company is

currently experiencing a substantial, serious, actual, and real loss due to
the economic effect of the COVID-19 pandemic on the tenancy of the
company’s Marvin Plaza Building, with more or less 70% have opted to
terminate their respective lease contract;

3. As of this date, the tenancy rate of the building remains at

33%, and as a consequence, the cash flow of the company continuously
diminished, affecting further the operational expenses of the company,
including ours but despite its depleted cash flow, the company has never
failed in giving me my salaries and all benefits due to me;

4. I genuinely appreciate and am thankful for the efforts of the

company and its compassion towards me and my workmates;

5. In recognition of the employees' economic challenges,

particularly this month of June, the company has decided to advance
the release of salary of its employees;

6. Thus instead of receiving my salary (for the cut-off period of

26 May 2023 to 10 June 2023) on 15 June 2023, I acknowledge that on
_______________, 2023 I received the same in full amount, less of any
deduction;

7. That considering receipt of my salary in advance, I hereby

grant consent and warrant the company to deduct any legal, and valid
deductions on my succeeding salary (for the cut-off period of 11 June
2023 to 25 June 2023), particularly any of my absences or tardiness
committed within the cut-off period covered by the salary I received in
advance;

8. In the event that I committed unauthorized absences or

reported late for my duty, during the cut-off period covered by the salary
I have received in advance, I further acknowledge and warrant the
company to impose upon me necessary disciplinary actions, including
but not limited to termination or suspension;

9. I finally declare that I have read and understand this

document and the release, waiver, and quitclaim hereby given are made
willingly and voluntarily and with full knowledge of my rights under the
law.
